Community Based Supports are services that provide direct support and assistance for participants in or out of participant’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. These supports are delivered one-on-one with a participant. A few examples of Community Based Supports are:
- Support from staff to attend an event or attend a class
- One-on-one tutoring
- Support from staff to assist in activities such as: daily living, ordering off a menu, purchasing, and accessing activities in the community
